Anyone know the answer to this question? Is an old 500LW with the Sierra STC legal to fly SP like a 501SP without a waiver?

I have never seen the Sierra STC to say definitely, but I doubt it. At one time Cessna offered a sex change STC that would change a 500 to a 501. They no longer offer it. I believe that included the assignment of a new type and SN. Without digging into the aircraft's history and paperwork I doubt it.

A related question for the Jet Guys....
For private part 91 flying by an owner/pilot. If I remember correctly the Single Pilot STC required a 2nd class Medical for hte Pilot.
Do all the single pilot Jets all require a 2nd class physical, or is 3rd class sufficient?

Paul Single pilot jets like the Citation 501 & 551 & 525 & 510, Eclipse, Premier, Phenom only require a 3rd class medical. The Single Pilot Wavier (SPW) needed to fly a Citation 500/550/560 single pilot depends on which STC you get. Most require 1st or 2nd class medicals. I have heard Front Range Executive Flight Training and CAE/Simuflite may not. You would need to call them and check.
